i usually check flights (i use JAL directly, but travelocity and other flight searching sites are fine), then hotels (i use asiarooms.com cuz i want things done easy). these days there's an easy NEX+suica card combo that can get you on the shinkansen to the tokyo + the suica card that you can ride on all JR and metros by an easy swipe sensor thing (like the octo card in HK). you just have to buy it at the lower level of the airport where the subways are.
if you want to get the JR rail pass, you have to pay for it at a travel agency in the states (they give you a voucher) that sells it and pick it up at the airport in japan (the same place as the new shinkansen + suica card deal...it's a travel center for foreigners)
if you choose a hotel with an airport bus though, you can just take an airport limo bus to your hotel...
